If your child is uninsured and you have not applied for Medicaid through a hospital, or a welfare office, we ask that you apply through your welfare office for Medicaid (Department of Health Care Financing and Policy). If your child has not recieved their Medicaid card, please bring in the stamped copy of your application paperwork. Pending Medicaid patients will be seen for a copay of $40 per visit. Should you not have that paperwork with you, you will be required to pay in full at the time of visit. A 50% discount will be applied to your bill. If you cannot pay that amount we have a hardship application process. You will be required to bring in your W2 and pay-stubs to show proof of hardship. You will not receive the 50% discount, but will be able to pay in installments.
